New Coronavirus Vaccine "University-based Inoculation" (3rd additional dose) Special Page
If you wish to be vaccinated at Hosei University, please follow the instructions below to make an appointment.
(1) First, please check the "Vaccination Overview" page here to confirm who is eligible for the vaccination, the type of vaccine, and the schedule.
(2) At the time of application, you must have received a vaccination ticket for the additional vaccination from your local government. (If you do not have a vaccination ticket, you will not be able to receive the vaccination at the main conference center.)

March 18 (Fri.) Start accepting reservations (Deadline is 2 days before each vaccination date)
(1) Access the "Web Reservation Site
*Faculty, graduate students, and faculty members will receive a Web bulletin board (Hoppii), and staff and other persons related to the University will receive a "Simple Operation Guide" containing the URL of the "Web Reservation Site" and the "Venue Code" necessary for logging in by e-mail, etc. Please confirm the URL and "Venue Code" necessary for logging in.
For School of Correspondence Education students, instructions for accessing and entering the Web Reservation Site are posted in the "Notices from the University" after logging in to the "Web Study Service".
(2) Please follow the "Simplified Operation Guide" to make a reservation.
(1) I want to cancel my reservation due to illness or other unavoidable circumstances.
Please cancel your reservation through the "Web Reservation Site" and make a new reservation.
(2) I want to cancel my reservation at the main conference venue because I think I will be able to take the seminar earlier at another venue (a venue set up by the local government, etc.).
Please be sure to cancel your reservation for the main conference center. Please follow the cancellation procedure described in (1) above.
Cancellation or Change of Appointment
Reservations can be cancelled or changed up to two days prior to the vaccination date. Cancellations or changes after that time will not be accepted in principle. Please do not cancel for personal reasons other than unavoidable circumstances (e.g., fever, illness, or death of a family member on the day of the vaccination). We ask for your understanding and cooperation in this matter, as strict control and use of vaccines is necessary.
(1) Filling out the preliminary checkup sheet
Please be sure to fill out the preliminary checkup sheet enclosed with the vaccination ticket, which you will receive from the municipality where you are registered as a resident, in advance. Please fill in your body temperature taken just before you leave your home on the day of the vaccination.
Please make sure to fill in your address after confirming that it is the address listed on your resident registration card. Please note that the address may not be the same as your current address if you are staying at a boarding house, etc.
If you have lost (or defaced) the medical checkup sheet
The handling will differ depending on the type of the preliminary checkup sheet received from the local government.
a. (Pattern 1) In the case of a preliminary checkup slip with an integrated vaccination ticket
Please contact the local government immediately for reissue.
b. (Pattern 2) In case the inoculation ticket and preliminary checkup sheet are separate
If you have the inoculation ticket but have lost (or defaced) the preliminary checkup sheet, please download the form from [here], print it out, fill it out, and come to the hospital. When printing, please do not print on both sides of the paper. Please be sure to print the form in A4 size with "100% magnification" or "actual size".
(2) Please make sure to get enough sleep on the day before the vaccination and eat a good breakfast on the day of the vaccination. It has been reported that adverse reactions are more likely to occur if you are not in good condition on the day of the vaccination.
(3) Please read through this pamphlet (published by Takeda Pharmaceutical Company Limited).
(4) Please access this page and check for any urgent notices (please check before leaving home on the day of the vaccination as well).
Please bring the following items to the venue. If you do not have any of the four items from a to d on the day of the vaccination, you will not be able to receive the vaccination even if you come to the venue. We may ask you to make an appointment for another day.
Please note that the pattern of documents from b to d differs from municipality to municipality. Please check the contents of the documents sent by your municipality carefully.
Pattern 1: In the case of a preliminary checkup sheet with an integrated vaccination ticket ⇒ "b" Vaccination ticket + "c" Preliminary checkup sheet and "d" Vaccination Certificate
Pattern (2): In the case of a preliminary checkup sheet that is separate from the vaccination ticket ⇒ "b" Vaccination ticket + "d" Vaccination certificate and "c" Preliminary checkup sheet
a Student/faculty/employee ID card or personal identification (driver's license, health insurance card)
b Vaccination ticket for the third vaccination received from the local government
*If your current address differs from the address on your resident registration card, the vaccination ticket will be delivered to the address on your resident registration card (the address of your guarantor, etc.).
*In case of pattern (2), please do not peel off the vaccination ticket from the sticker base, but bring the entire base with you.
c. Pre-medical checkup sheet (completed)
d. Immunization Certificate
e Printout of the appointment completion e-mail
(1) Please bring the items listed in "6 What to bring on the day of your visit.
(2) Please come dressed in clothes that allow you to show your shoulders so that the vaccinations can be administered smoothly.
(3) On the day of the vaccination, please check this page for directions to the venue and come to the venue (Kudan North School Building).
(4) After the vaccination, please stay at rest.
(5) If you experience any symptoms that you think may be an adverse reaction after the vaccination, please refer to the instructions here (published by the Ministry of Health, Labour and Welfare).
