Reservation Procedure and Steps to the Day of Vaccination

New Coronavirus Vaccine "University-based Inoculation" (3rd additional dose) Special Page

If you wish to be vaccinated at Hosei University, please follow the instructions below to make an appointment.

1 Notes on the Appointment

(1) First, please check the "Vaccination Overview" page here for information on who is eligible for the vaccine, the type of vaccine, the schedule, and other details.
(2) Only those who have never received the novel coronavirus vaccine may apply for the vaccination at the conference venue. Those who have already received the first vaccination at another venue (vaccination conducted by the local government, etc.) are not eligible to apply.
(3) Those who have received the first vaccination at the main venue must also receive the second vaccination at the main venue.
(4) Reservations for the second vaccination will be automatically made for the same day and time four weeks after the completion of the first vaccination. Please make an appointment for the first vaccination after securing the schedule for the second vaccination in advance.

2 Reservation Period

Monday, August 23, 12:00 (noon) - Wednesday, September 1, 24:00 (JST)
However, those who fall under the following categories will be able to make advance reservations between 10:00 and 12:00 on Monday, August 23. If you do not fall under this category, please do not make reservations until 12:00.
Those who are 65 years old or older
Those who have an underlying medical condition (for a specific definition of "underlying medical condition," please refer to p. 7 of the 24th Subcommittee on Countermeasures to Combat New Coronavirus Infectious Diseases (Cabinet Secretariat) meeting held on February 9, 2021).
Reservations can be made up to two days before the vaccination date (four days before August 30 only).

3 Reservation Procedure

(1) Please access the "Vaccination Reservation Site
  *Please check the Web bulletin board (Hoppii) for undergraduate and graduate students and faculty members, and the "Simple Operation Guide" containing the URL of the "Web Reservation Site" and the "venue code" necessary for logging in will be sent to staff members and other persons related to the University by e-mail.
For School of Correspondence Education students, instructions for accessing and entering the Web Reservation Site are posted in the "Notices from the University" after logging in to the "Web Study Service".

(2) Please follow the "Simplified Operation Guide" to make a reservation.

4 In such cases

(1) I want to cancel my reservation due to illness or other unavoidable circumstances.
Please cancel the first vaccination through the "Web Reservation Site" and make a new appointment.
For the second vaccination, please call the call center. Please follow the instructions given by the call center to change to a different date and time.

(2) I would like to cancel my reservation at the main conference site because it seems that I can receive the vaccination earlier at another site (a site set up by the local government, etc.).
Please be sure to cancel your reservation for the main conference center. Please follow the cancellation procedure described in (1) above.

Cancellation or Change of Appointment
Reservations can be cancelled or changed up to 2 days prior to the vaccination date (4 days prior to August 30 only). Cancellations or changes after that date will not be accepted in principle. Please do not cancel for personal reasons other than unavoidable circumstances (e.g., fever or poor health on the day of the vaccination, personal illness, etc.). Due to the strict control and use of vaccines, we ask for your understanding and cooperation.

(3) I would like to inquire about how to use the "Web Reservation Site" or about reservations.
A dedicated call center will be open during the following period. Please contact us by phone.

  • Open: Weekdays except Saturdays, Sundays, and holidays from August 23 (Mon.) to October 2 (Sat.) (except for October 2 (Sat.))
  • Hours: 9:30 a.m. to 4:30 p.m.

 *The telephone number of the call center will be provided in the "Simple Operation Guide.

(4) All the reservations for the desired date and time have been filled.
The availability of the appointment slots changes constantly due to cancellations and other reasons. If a cancellation occurs, the appointment slot will be released and you will be able to make another appointment.

5 Things to do before the day of vaccination

(1) Fill out the preliminary consultation form
Please download the form from here and be sure to fill it out in advance. Please fill in your body temperature taken just before leaving home on the day of the appointment.
Please make sure to enter your address as it appears on your resident registration card. Please note that your address may not match your current address if you live in a rooming house, for example.

(2) Please have the vaccination ticket ready at hand. (If you have not received your vaccination ticket from your local government, you will need to submit it to the university as soon as you receive it.)

(3) Download the Vaccination Record Form here, and be sure to fill in your name, address, and date of birth in advance.

(4) Please make sure to get enough sleep the day before the vaccination and eat a good breakfast on the day of the vaccination. There are reports that adverse reactions are more likely to occur if conditions are poor on the day of the vaccination.

(5) Please read through this pamphlet (published by Takeda Pharmaceutical Company Limited).

(6) Please access this page and check for any urgent notices (please check before leaving your home on the day of the vaccination as well).

6 Things to do on the day of vaccination

(1) Please bring the following items to the venue
 a Student/faculty/staff/employee ID card or personal identification (driver's license, insurance card)
 b. Pre-Checkup Form (completed)
 c. Inoculation record form (completed)
 d. Inoculation ticket (if received from local government)
 e Printout of the appointment completion e-mail

(2) Please come dressed in clothes that allow you to show your shoulders easily so that the vaccination can be administered smoothly.

(3) On the day of the vaccination, please enter the building from Fujimizaka Gate (on the side of Tokyo Postal Service Hospital).

(4) After the vaccination, please read the precautions here (issued by the Ministry of Health, Labor and Welfare) carefully and stay at rest.

(5) If you experience any symptoms that you think may be an adverse reaction after the vaccination, please refer to this page from the Ministry of Health, Labour and Welfare to find out what to do. 7 (Only after the first vaccination) To be done on or after the day following the vaccination

(1) An appointment for the second vaccination will be automatically scheduled for the day after the completion of the first vaccination. (As a general rule, the second vaccination is scheduled for "the same day and time four weeks later.) Once the appointment is completed, an e-mail will be sent to you, so please confirm the date and time of your second appointment.

(2) "Things to do before the day of the vaccination" and "things to do on the day of the vaccination" are the same as for the first vaccination.
